If you do not have a credit or debit card, or need alternative payment arrangements, please call 360-545-3356 to make your appointment on the phone. 

Before your visit, please fill out the intake forms sent to you, so that we have the required consents to have our visit, as well as important information to inform our care plans, customized just for you & your family's needs. Thank you!
